DECREE

about the patterns of documents under the law on the citizenship of the Czech Republic



Ministry of the Interior shall be determined according to section 79 of the Act No. 186/2013 Coll.

the citizenship of the Czech Republic and amending some laws (law on

the citizenship of the Czech Republic), (hereinafter referred to as the "Act"):



§ 1



The Charter of the acquisition of citizenship of the Czech Republic the granting of an

§ 4



Certificate



Specimens of the certificates under section 43 of the Act, pursuant to section 43 of the Act certificate in which

the request shall indicate the information about when, how and by what

law natural person of the Czech or Czechoslovak State

citizenship acquired or disposed, or also about the fact that on a specific date or

within a certain period was a Czech citizen or the Czechoslovak State

citizen, or that the individual is a citizen of the United

the Republic continuously from a certain date to the time of issue of the certificate, and

certificate under section 48 of the Act are set out in annex 4 to this

the Decree.



§ 5



Forms



(1) the forms of documents, specimens of which are given in annexes 1 to 4

This Decree, are strictly zúčtovatelnými forms with securing

the elements against their abuse, counterfeiting and alteration. Each form is

equipped with a letter indicating the series and each series number šestimístnou

from 000001 to 999999. After the exhaustion of numbers given for each series follows

form of the series provided for by adding another of the same letter, identifying the

a series of šestimístnou and digit from 000001 to 999999.



(2) the forms are printed on A4 paper from bezdřevém 100%

pulp, 110 g/m2, color PANTONE 365, without optical brightener with full

chemical protection.



(3) Securing elements of the forms are in addition to the full chemical protection

the paper's own unique and unmistakable running watermark lion in the shield

and lime leaves.



§ 6



Regulation (EEC)



Repeals the Decree 425/2013 Coll. on the models of documents under the law on

the citizenship of the Czech Republic.



§ 7



The effectiveness of the
