on the determination of requirements and designs for the exercise of State supervision credentials,
State professional supervision, the Chief of State supervision and the Chief of the State
professional supervision in the transport sector in the form of licence and amending related
decrees
The Ministry of transport shall be determined according to § 46 para. 2 Act No. 13/1997 Coll.
on road traffic, as amended by Act No. 102/2000 Coll., Act No.
80/2006 Coll., Act No. 97/2009 Coll., Act No. 347/2009 Coll., Act No.
152/2011 Coll., Act No. 196/2012 Coll. and Act No. 64/2014 Coll., pursuant to §
paragraph 41. 2 Act No. 111/1994 Coll. on road traffic, as amended by law

DETERMINATION OF THE REQUIREMENTS AND DESIGNS THE CREDENTIALS IN THE FORM OF LICENCE
§ 1
The elements and patterns of credentials to State supervision and high performance
State supervision in matters of road infrastructure in the form of licence
(To section 41, paragraph 3, of Act No. 13/1997 Coll.)
(1) a mandate to exercise State supervision and the Chief of the State supervision in the
matters of road infrastructure in the form of the card contains the
and) designation of the administrative authority which issued the credentials, which is listed
the text of the "Czech Republic",
(b)) a statement that the credentials to State supervision or high performance
State supervision in matters of road infrastructure,
(c) the number of credentials)
(d)) name and surname of an authorised person,
e) assignee/photography 35 x 45 mm, the lower edge of the
is přelepen a holographic přelepkou with the text "CZECH REPUBLIC",
f) date of issue of the credentials
g) information about the permission of an authorised person,
h) signature of person authorized to issue a credential,
I) the imprint of the stamp of the administrative authority that issued the credential.
(2) a mandate to exercise State supervision and the Chief of the State supervision in the
matters of road infrastructure in the form of card is double-sided paper
card with dimensions of 70 x 100 mm, which is packed into the transparent laminating
foil/75 x 105 mm. card is equipped with the safety features,
which are the embossed background, giloš, micro lettering, in the UV spectrum of luminous
color, ceninový paper with a watermark positiv x negative, that contains two
types of invisible threads of the illuminating in the UV spectrum, viewing brand and
the numbering of the glowing green with black in the UV spectrum.
(3) the patterns of credentials to State supervision and the high performance state supervision
in matters of road infrastructure in the form of licence are set out in annex
No 1.
§ 2
The elements and designs for the performance of the State's credentials, professional supervision and
the high State supervision in road transport in the form of licence
(Section 34, paragraph 5, of law No. 111/1994 Coll.)
(1) a mandate to exercise State supervision and the State vocational high
supervision in road transport in the form of the card contains the

vehicles in the form of licence
(Section 54 (3) of the law No. 247/2000 Coll.)
(1) a mandate to exercise State supervision and the Chief of the State supervision in the
matters of competence to drive motor vehicles in the form of
the card contains the

UV spectrum of luminous color, ceninový paper with a watermark positive x
negative, consisting of two invisible fibres glowing under UV
spectrum, viewing brand and numbering in black flashing green
in the UV spectrum.
(3) the State performance credentials Designs professional supervision and Supreme
State vocational supervision delegated responsibility in matters of
the road in the form of licence are set out in annex 4.
§ 5
The elements and patterns of credentials to State supervision and high performance
State supervision in matters of runways in the form of licence
(Section 58, paragraph 5, of law No. 266/1994 Coll.)

matters of runways in the form of the card contains the

Aviation in the form of licence
(§ 90, paragraph 4, of Act No. 49/1997 Coll.)
(1) a mandate to exercise State supervision in civil aviation in the form of

§ 10
Transitional provision
The existing credentials to exercise State supervision over local roads and
public-purpose communications in the form of a licence issued
in accordance with the specimen in annex No 9 of Decree No. 104/1997 Coll., as amended by
effective before the date of entry into force of this Decree shall remain in
validity until 31 December 2004. December 2017.
PART THREE
Amendment of the decree implementing the law on Civil Aviation
§ 11

1. in article 2, paragraph 3, including footnote # 4 be deleted.
Paragraphs 4 and 5 shall become paragraphs 3 and 4.
