DECREE



The Czech Mining Authority



of 3 July 2003. January 1995



for permissions to mining activities and the activities carried out by mining

in a way, as well as to the design of objects and facilities, which are

part of these activities

The Czech mining Office lays down pursuant to § 5 para. 2 of the Act of the Czech National Council

No. 61/1988 Coll. on mining activities, explosives and the State mining

the Administration, as amended by the Act of the Czech National Council No. 425/1990 Coll., Act

The Czech National Council No. 542/1991 Coll. and Act No. 171/1993 Coll.:



§ 1



(1) permission to mining activities and the activities carried out by mining

in a way, as well as the planning and designing of objects and devices that

are part of the mining activities, or activities carried out by mining

(hereinafter referred to as the "permissions"), may be issued to the legal or natural

to the person who proves that the competence itself or through

(3) the Permission may be exercised over the entire territory of the Czech Republic.



§ 2



(1) legal or natural person in the application for authorisation shall be



a trade name, a) ^ 2), where appropriate, the name and surname,



(b)) the seat of the place of business or, where appropriate, place of residence,

(c) the identification number, if applicable), date of birth,

(2) the application shall be accompanied by documentary, or certified copies thereof,

paragraph 1.

§ 3



(1) permission shall indicate:

(b)) the seat of the place of business or, where appropriate, place of residence,

d) objects,

(2) the district mining Office shall send a copy of the permission to the competent tax authorities and

§ 4



(1) Permission shall cease to



and) the expiry date, if the permission is given,



(b) dissolution of the legal person or the) death of a natural person,



(c)) date of expiry of the certificate of fitness to operate

or termination of the employment relationship or other similar relationship

§ 5

§ 6



This Decree shall enter into force on 1 January 2000. March 1995.
