According to the law on conflicts of interest



The Justice Department determined in accordance with § 12 para. 5 of law No.

159/2006 Coll. on conflicts of interest (hereinafter referred to as the "Act"):



§ 1



The structure and format of the form, which serves a notice under section 9 to

11 and § 12 para. 2 of the Act are set out in the annex to this Decree.



§ 2



This Decree shall enter into force on 1 January 2000. January 1, 2007.
