on the system of the safety of the operation of railways and railway transport, and procedures

upon the occurrence of incidents in rail systems

The Department of transportation shall determine pursuant to section 66 paragraph 1. 1 Act No. 266/1994 Coll.

on rail, as amended by law No 23/2000 Coll., Act No. 77/2002 Coll.

Act No. 103/2004 Coll. and Act No. 182/2006 Coll. (hereinafter the "Act") to

the implementation of § 22 para. 5, § 23a para. 7, § 34 h of paragraph 1. 9, § 35 para. 3, §

49e para. 6 and § 53b para. 1, 2 and 5 of the Act and under section 66 paragraph 1. 5 of the Act:



§ 1



The subject of the edit

§ 2



The elements of the system of ensuring the security of the operation and the operation of

the rail transport



(1) the system of ensuring the security of the operation and the operation of

d) establishing safety objectives and measures to carry out continuous improvement

the system of ensuring the security of the operation of railways or railway transport.

§ 3

national or regional path contains the evaluation of the State of the system

securing the operation of railway transport, at least:



and information on the implementation of security) objectives, which the carrier has

Annex No. 4 of this order,

could lead to an accident or a serious accident.



§ 5



Model application for issue of the certificate of the carrier on the national or

regional and the model of the carrier on the national or

regional

§ 6



The contents of the annual report on the activities of the Office Overhead

the activities of the operators of the railways and the national and regional carriers

in the rail transport on these rail systems, carried out by the Railway Authority.



§ 7



The procedure for the formation of an emergency in railway transport



(1) the operator of a runway and the carrier shall establish separate or joint

reporting a workplace to ensure announcement of the formation of an emergency

railway transport, the operator and the carrier ensures that each

§ 8

§ 9



Measures at the site of the emergency



(1) to detect the causes and circumstances of an incident arising out of

§ 10



Jurisdiction to determine causes and circumstances of an incident



(1) the operator and the carrier according to the activities carried out in the framework of the

g) data, the causes and circumstances of an incident,

activation of the integrated rescue system plan and medical

emergency services,



(h) determine the responsibility for the creation of) an incident with an indication of

the provisions of the laws, internal rules and harmonised or intended

technical standards, which have not been complied with,



even) if the incident caused by a failure of the human factor,

assessment of the health status and personal situation of the participants of the event,

that had an effect on the incident, including physical and psychological

stress,



j) proposal of corrective measures to prevent incidents, including

proposal deadlines,



to) date and place copies of the evaluation, the name and signature of an authorised person,



l) signature of the statutory body of the rail operator or carrier in accordance with

of substantive jurisdiction.



(6) the assessment of causes and circumstances of a serious accident or of an accident

be sent to the operator or carrier under substantive jurisdiction within 60

days from the occurrence of an extraordinary event of railway inspection and the railway or railway administrative

§ 11



Determination of serious accidents in railway transport, accidents and risk for

the detection of causes and circumstances of Railway inspections



(1) Railway inspection detects the causes and circumstances of serious accidents

(article 49, paragraph 2, of the Act) in the railway transport on the national, the track

§ 12



Report on the results of the investigation of causes and circumstances of extraordinary

events



A report on the results of the investigation of causes and circumstances of extraordinary

the event will draw up a Railway inspection in accordance with the specimen in annex # 7

of this order. The report shall be drawn up as a rule, within 6 months from the inception of the

extraordinary events; If an expert opinions or expert

§ 14



Common provisions



(1) If an emergency vehicle when operating overhead on

1. requirements for safety assurance systems

(f) training of staff and programs) systems which ensure the maintenance of

employee qualification and the corresponding level of fulfilment of the tasks,



g) measures to ensure sufficient information is provided in the operator's

the runway or the carrier and if necessary between the carriers using the same

route of the track,



h) procedures and patterns for documenting security of information and

establishment of the procedure for checking the transmission of the most important

security information,



I) procedures to ensure that they are serious accidents, accidents, emergencies and other

the events affecting the safe operation of railways and railway transport

notified, assessed their causes and are analysed and that are

taken the necessary preventive measures,



j) plans of intervention warnings and the transmission of information in the event of an emergency

situations, which are agreed upon with the competent public authorities,



k) provisions on the implementation of periodic internal controls system

Designation of the petitioner:



Registration overview provides common safety indicators, which

they are:



I. indicators relating to accidents



1.1 Total and relative (to train 1 km) the number of accidents:



and the reduction of the train to the railway) vehicle, IE. front, rear, or side collision

between the parts of the train, and part of another train or vehicle overhead,



(b) reduction of the train with an obstacle) gauge, i.e.. collision between parts

The train mile means the unit of measure representing the movement of a train

at a distance of one kilometre. If known, actually

driving distance, otherwise, it's the standard network
