concerning the register of accidents of children, pupils and students

The Ministry of education, youth and sports, determined in accordance with § 29 para. 3
Act No. 561/2004 Coll. on pre-school, primary, secondary, higher
vocational and other education (the Education Act):
§ 1
Book-related injuries
(1) in the book of accidents are recorded all injuries to children, pupils and students (
"the accident") that occurred during activities referred to in section 29 para. 2
the Education Act, and not later than within 24 hours from the moment when the school
or the educational establishment of the accident.
(2) the following shall be entered in the book of accidents
and the serial number of the accident)
(b) the name or names), surname and date of birth of injured,
(c) a description of the injury,)
(d)) event description in which the accident occurred, including the date and place
events,
e) whether and by whom it was injury treated,
(f) the signature of the employee of a legal person) carrying out activities of a school or
the educational establishment, which has made an entry in a book of injuries,
g) for more details, if they are needed to draw up a record of the accident.
(3) personal data which are included in the book of injuries, can be
processed only for the purpose of registration of accidents at work, where appropriate, as a basis for
copies of the record.
§ 2
Record of the accident
(1) the record of the accident shall be made out, the school or educational establishment, if it is a
and that results in injury) was the absence of the child, pupil or student
(hereinafter referred to as "disciple") in a school or a school that spans at least to

(3) at the request of the legal representative of the pupil, an adult student or institution
referred to in paragraph 4 of the school or educational establishment, without delay, shall draw up a record
and the injury that is not listed in paragraph 1.
(4) School or educational establishment shall be drawn up of the accident that is not listed in the
paragraph 1, record, if it is likely that the pupil will be given
compensation for the pain and make it more difficult for social application caused by the injury.
(5) a copy of the record passes the school or educational establishment

§ 3
Reporting an accident
(1) the injury of a minor pupil school or educational establishment shall, without

the locally competent service of the police of the Czech Republic.

reporting to the insurance undertaking which is a school or educational establishment insured
for the case of its responsibility for the damage caused to life and health of the pupils,

(4) School or educational establishment without undue delay shall submit reports on the
injury pursuant to § 2 (2). 1 and 4 also the competent district Inspectorate
work, or to the relevant district mining authority, if the injury
occurred in the practical teaching of the pupils of secondary schools or practical
prepare students for the colleges.
§ 4
Sending a record of accident
(1) the record of the accident according to § 2 (2). 1 (b). (a)) 4 sends school

on the fifth day of the following month,

(2) a record of a mortal accident sends school or educational establishment to 5

§ 5
