PART THE FIRST
IDENTITY CARDS
§ 1
Pattern ID card machine legible characters and with a contact
an electronic chip
(1) the model of national identity card, the machine-readable data and with a contact
an electronic chip is listed in annex 1 to this notice.
(2) the machine-readable data shall be entered in the specified zone in the bottom of the

§ 2

§ 3
Pattern ID card without a machine-readable data
(1) the Specimen ID card without a machine-readable data is listed in the
Annex No. 3 to this notice.

specific records on the back side of the identity card.
§ 4
Marked part of the identity card
(1) the marked parts of the identity card, a specimen of which is given in the
Annexes 1 and 2 to this Ordinance, means both its upper corners,
colour-marked on the back of (A) and (B).
(2) when you report a change of residence is separated part marked
(A).
(3) when you change the data being written to the card, in addition to the changes
data on the place of residence referred to in paragraph 2, shall separate the part marked with
(B).
(4) when you report a change of residence for identity card
issued to 30. April 1993 separates the lower-right corner of the front plates, for
ID card issued after 30. April 1993, that does not contain

§ 5
Forms a confirmation of identity card and a certificate of change in the particulars
being written to the card
(1) the specimen certificate of identity card is given in annex No.
4 to this notice.
(2) the specimen certificate of change of the data being written to the civil
certificate is given in annex 5 to this Decree.
§ 6
Application for the issue of identity card
(1) the pattern of electronically processed applications for identity card
the machine-readable data and with contact electronic chip is
listed in annex 6 to this Ordinance.
(2) the model electronically processed applications for identity card
the machine-readable data is given in annex 7 to this Decree.
(3) the specimen application for issue of identity card without a machine
readable data (hereinafter referred to as "application") is given in annex 8 to this
the Decree.
§ 7
The requirements for the technical construction photos for the issue of the civil
licence
(1) the Photographs or other visual recording ("photos")

§ 8
The conditions and the technical way of sending digital photos
through a data box of the photographer
Digital photos taken by the citizen for the issue of identity card
a photographer is sent to set up a data box separately from the Ministry of

identity cards, using the electronic form published on

§ 9
Technical parameters of digital photography
A digital photograph of the citizen must, in addition to the requirements referred to in § 7 para.
2 to 5 must meet the following technical parameters
and the size of the data file containing) the photograph, a maximum of 90 kB,
at least 50 kB,
(b)) that contains the data file format photo jpg/jpeg/jfif with
a .jpg file ^ 3),
(c)) the width of 2.6 cm photos, 600 DPI resolution,
d) 3.4 cm height, 600 DPI resolution and
e) color depth 24-bit, RGB color model.
PART TWO
TRAVEL DOCUMENTS
§ 10
The pattern of the Passport is machine-readable data carrier and data with
biometric data
(1) the model passport is machine-readable data carrier and data with
biometric data is given in annex 10 to this Decree.
(2) the machine-readable data shall be entered in the specified zone in the bottom of the
the data page of the passport specified in annex 10 to this

(3) the data carrier with the biometric data is an integral part of a data access page

§ 11
The pattern of the passport without a machine-readable data without the data carrier with the
biometric data
(1) the model of a passport without a machine-readable data without the data carrier
with the biometric data is given in annex 11 to this Decree.
(2) the Unabridged names, or names, and last names are entered in the

§ 12
The Passport is machine-readable data carrier and data with
biometric data contains 34 pages and passport without a machine
readable data, and without the data carrier with biometric data contains 32

§ 14

PART THREE
TRANSITIONAL AND FINAL PROVISIONS
§ 17
Transitional provisions
(1) the application forms for the issue of identity card without a machine
readable data, confirmation of the identity card, the issuing of
the passport without a machine-readable data without the data carrier with the
biometric data, application for issue of the licence, the application for
the issue of travel documents issued on the basis of international agreements,
issued under Decree No. 642/2004 Coll., in the version in force before the date of application
the effectiveness of this Ordinance, may be used until 31 December 2004. December
2016.
(2) without passports machine readable data, and without the data carrier with the
biometric data in accordance with the model laid down in annex 9 of Regulation No.
642/2004 Coll., in the version in force until the date of entry into force of Decree No.
417/2006 Coll., can be issued until 31 December 2004. December 2012.
(3) the forms for application for issue of the licence issued in accordance with the model
