of 24 July 2003. August 2015



about the extent of processing information to the public, reports on the major

the accident and the final report of the occurrence and consequences of a major accident



Ministry of the environment shall determine pursuant to § 54 para. 1 of law No.

224/2015 Coll. on the prevention of major accidents caused by selected

hazardous chemicals or chemical mixtures and amending the Act

No. 634/2004 Coll., on administrative fees, as amended,

(law on the prevention of major accidents), (hereinafter referred to as the "Act") to implement section

35 para. 6 and § 36 odst. 5 of the Act:



§ 1



The subject of the edit

§ 2

§ 3

§ 4

§ 5

§ 6

listed in part A of this annex and includes the following information:

Instructions for filling out the form of the final report of the occurrence and consequences of serious

crash



Section A: DESCRIPTION Of MAJOR ACCIDENT



Note 1: a type of severe accident
