waste and amending certain other acts, (hereinafter referred to as "the Act"):



PART THE FIRST



APPLICATION FOR APPROVAL TO OPERATE A DEVICE TO USE, REMOVING THE COLLECTION

OR THE PURCHASE OF WASTE AND REQUEST FOR CONSENT TO THE MANAGEMENT OF HAZARDOUS WASTE

[To section 14, paragraph 5 (a)) and (b)) and article 16, paragraph. 5 of law]



§ 1



The elements of the request for approval to operate a device to use,

delete, collection or purchase of waste



(1) an application for permission to operate a device to use,

delete, collection or purchase of waste (hereinafter referred to as "equipment") contains:



and the business name or name), legal form and registered office, if the applicant

a legal person; name and last name, business name, domicile and the place of

the business, if different from the residence, where the applicant is a natural person,



(b) the applicant's identification number) has been allocated, the

extract from the commercial register or a copy of the incorporation documents of the applicant,



(f)) name, surname, arranging, and evidence of his vocational

eligibility under section 15(2). 5 of the Act, if the applicant has, under section 15 of the

law to determine the waste Manager,



(g)) the name, purpose and technical description of the device including all equipment

related, a description of the technological process in the waste

h) a list of species of waste according to waste catalogue, which will be in the

device usage,



I) description of the access routes to the device in relation to individual species

(j)) method of storage of waste in the facility,

about the details of the handling of the car.



(2) the request to grant consent to the operation of the waste dump contains the

§ 2



The elements of the request for approval to the management of hazardous waste



A request for consent to the management of hazardous wastes include:

§ 3



For the purposes of this part of the Ordinance means

§ 4



General requirements for the use of the device and the removal, collection and purchase

waste



(1) the use of devices, the removal, collection or purchase of waste (hereinafter

"the equipment") shall comply with the requirements laid down by specific legal

provisions on the protection of the environment and people's health ^ 3) and must be

operated and equipped so as to avoid pollution of the

§ 5



Waste collection



(1) as a means to muster a hazardous waste may be used

§ 6



Concentration of waste

(3) Identification of natural persons referred to in paragraph 2 means the findings

Technical requirements on the disposal of waste oils



section 13



(1) the types of waste according to the waste catalogue, which is considered to be waste

THE CONTENTS OF THE ANNUAL REPORT ON THE PERFORMANCE OF THE DUTIES COLLECTED FOR THE PAST

CALENDAR YEAR (section 38 of the Act)

(1) the annual report on the performance of the duties collected for the past

calendar year contains:

similar waste from non-productive activities of legal entities and natural persons

ways of dealing with them for the past year, according to annex No. 20.



(5) the Information referred to in paragraphs 1 to 4 shall be sent in accordance with Annex No. 20 agents

the obligation at the same time as the originators of the waste, electronically in

The method of reporting facilities for waste management



(1) operators of installations for the disposal of waste (with the exception of landfills

waste) and operators of installations for the recovery of waste and operators

the device referred to in section 14, paragraph. 2 of the Act shall be sent to the competent municipal

Annex 27 is used for sending data on the carriers of waste.



Carrier-is a legal or natural person, which provides transportation
