(a) The schedule of fees shall be as follows:
  (1) request for placement on the registry--$500;
  (2) initial application for placement on the registry --$120;
and
  (3) annual reapplication (any category) for placement on the
registry --$120.
(b) All fees are nonrefundable.
(c) An applicant whose personal check for placement on or reapplication
to the registry is not honored by the financial institution may reinstate
the application by remitting to the department a money order or cashier's
check for the amount within 30 days of the date of the applicant's receipt
of the department's notice. An application will be considered incomplete until
the fee has been received and cleared through the appropriate financial institution.
An applicant who is reapplying will be removed from the registry if the amount
is not remitted within 30 days.
