§

4261. Lost, replacement, or free license certificate

(a) A person who

has lost a license other than a lifetime license or permanent license may

demand a lost license certificate from the agent of original issue. The fee

shall be $5.00 which the agent may retain. If the agent of original issue is no

longer selling licenses, the applicant may apply directly to the Department. If

available, replacement and free licenses may be obtained from a point-of-sale

agent or online at the State's website. If requested from a point-of-sale

agent, a $1.50 filing fee may be charged and retained by the agent.

(b) A person who

has lost a lifetime license or permanent license may obtain a new license upon

application to the Department, payment of a $5.00 fee and submission of proof

of identification. If available, replacement and free licenses may be obtained

from a point-of-sale agent or online at the State's website. If requested from

a point-of-sale agent, a $1.50 filing fee may be charged and retained by the
