RULE §477.8 Forms for Recording of Willed and Donated Bodies

(a) Member institutions operating a willed body program,
and institutions or individuals receiving donated bodies, including
those authorized under Health and Safety Code, Chapter 692A, shall
prepare separate forms for pre-death wills under Health and Safety
Code, Chapter 691 and post-death donations under the Anatomical Gift
Act, Health and Safety Code, Chapter 692A. A copy of such forms shall
be deposited, as a sample, with the secretary-treasurer.
(b) All Chapter 691 will forms and Chapter 692A donation
forms shall incorporate the following: "Complaints or inquiries regarding
a willed or donated body should be directed to the secretary-treasurer
of the Anatomical Board of the State of Texas. The name and address
of this individual may be obtained from the institution to which the
body was delivered."
