SERVICES

For Dental Care Services provided by the Division, materials

and supplies shall be furnished by the Division, subject to the availability of

funds.

10A NCAC 40C .0205       PARENTAL PERMISSION FORM

A child receiving dental care services must have a

permission form signed by the child's parent or guardian.  Parental permission

forms shall be available from the Division.

10A NCAC 40C .0206       INDIVIDUAL PATIENT TREATMENT FORM

(a)  Individual patient treatment forms are available from

the Division.

(b)  The patient treatment form shall be completed in ink

and signed and dated by the dentist.
