(a) Proper sponsorship and disclosure of each location
at which an originator is conducting regulated activities is required
to be on their Nationwide Mortgage Licensing System and Registry record.

(b) A licensee shall notify the Commissioner by filing
a license amendment through the Nationwide Mortgage Licensing System
and Registry of the following:
  (1) any change of address;
  (2) name change; or
  (3) a new, or changed company, organization, or assumed
name.
