MEMORANDUM ORDER NO. 745

REQUIRING SUBMISSION OF DULY ACCOMPLISHED DAILY TIME RECORD (CIVIL SERVICE FORM NO. 48), AFTER THE END OF EVERY MONTH.

Effective immediately, all officials and employees of this Office, except officials with the rank of Assistant Secretary and above, are hereby required to submit duly accomplished Daily Time Record (Civil Service Form No. 48) to the Administrative Office after the end of every month.

The time of arrival and departure appearing in the Daily Attendance Blotter required under Memorandum Order No. 6j59, dated January 22, 1979, shall be the one reflected in the Daily Time Record duly certified by the employee concerned and verified by his or her supervisor.

For purposes of audit, and until such time all employees of this Office will be using the bundy clock again, each head of office/primary unit shall submit a list of his employees with a summary of their individual services rendered for one whole month, indicating their leave of absence with and without pay, in a form herein prescribed (see attached sample). This form shall be submitted to the cashier who shall attach the same to the payroll of the office/primary unit concerned.

The payrolls without the herein required certification of services for the period covered, shall not be passed in audit and the salaries of affected employees shall be withheld until proper compliance herewith.

The salary of an employee who may fail to submit the herein prescribed Daily Time Record’ shall be withheld and shall not be released until proper compliance herewith.

The Administrative Office and the Finance Office shall see to it that this Memorandum Order is strictly implemented and complied with.
