Digital Switchover (Disclosure of Information) (Jersey) Order 2010
THE MINISTER FOR ECONOMIC DEVELOPMENT, in pursuance of Articles 2(6) and 4 of the Digital Switchover (Disclosure of Information) (Jersey) Law 2010[1], orders as follows –

1 Interpretation
In this Order, “the Law” means the Digital Switchover (Disclosure of Information) (Jersey) Law 2010[2].
2 Prescribed information
(1) For the purposes of Article 2(6) of the Law, information of the following description about a patient in a care home is prescribed as care home residency information –
(a) the patient’s title, given name and family name;
(b) any other name by which the patient is known;
(c) the address of the care home;
(d) the patient’s date of birth;
(e) the patient’s social security number;
(f) the title, given name and family name, and address, of another person (if any) appointed to act on the patient’s behalf;
(g) any other name by which the other person is known;
(h) whether the patient is eligible for help under the switchover help scheme;
(i) if the patient has died after the supply under Article 2 of the Law of care home residency information about the patient, the fact and date of the patient’s death.
(2) For the purposes of Article 2(6) of the Law, information of the following description about a person is prescribed as social security information –
(a) the person’s title, given name and family name;
(b) any other name by which the person is known;
(c) the person’s home address;
(d) the person’s date of birth;
(e) the person’s social security number;
(f) the title, given name and family name, and address, of another person (if any) appointed to act on the first person’s behalf;
(g) any other name by which the other person is known;
(h) whether the first person is eligible for help under the switchover help scheme;
(i) if the first person has died after the supply under Article 2 of the Law of social security information about the person, the fact and date of the person’s death.
(3) For the purposes of Article 2(6) of the Law, information of the following description about a person is prescribed as visual impairment information –
(a) the person’s title, given name and family name;
(b) any other name by which the person is known;
(c) the person’s home address;
(d) the person’s date of birth;
(e) the person’s social security number;
(f) the title, given name and family name, and address, of another person (if any) appointed to act on the first person’s behalf;
(g) any other name by which the other person is known;
(h) whether the first person is eligible for help under the switchover help scheme;
(i) if the first person has died after the supply under Article 2 of the Law of visual impairment information about the person, the fact and date of the person’s death.
3 Citation
This Order may be cited as the Digital Switchover (Disclosure of Information) (Jersey) Order 2010.
