84/2011 Coll. DECREE of 16 March 2011 amending Decree no. 296/2004 Coll., Implementing the Act on the Population Register and amending Decree no. 177/2000 Coll., Implementing the Population Register Act, the Law on Identity Cards and Travel Documents Act, as amended, as amended, the Ministry of the Interior, pursuant to § 25 point. a) and b) of the Act no. 133/2000 Coll., on register of population and birth certificate numbers and on amendments to certain Acts (Register of Population Act), as amended by Act no. 53/2004 Coll .: Article. I Decree no. 296/2004 Coll., Implementing the Act on the Population Register and amending Decree no. 177/2000 Coll., Implementing the Population Register Act, the Law on Identity Cards and Travel Documents Act, as amended amended, as amended by Decree no. 661/2004 Coll. and Decree no. 299/2006 Coll., is amended as follows: 1. The title of the Decree, the words' and amending Decree no. 177/2000 Coll., implementing the Population Register Act, the Law on Identity Cards and the Act on Travel Documents, as amended "are deleted. 2. In § 1, at the end of paragraph 1 the sentence "procedure under the second sentence shall not apply if a report or requested personal identification numbers referred to in paragraph 2 provided remote access via an electronic communications network providing a feedback communication with výdejovým place.". 3. In § 1 para. 3 point. a) the words "telecommunications network" is replaced by "electronic communications network". 4. § 2 para. 2 letter e) including footnote no. 7 reads: "e) in the case of adoption of the name or names of the adoptive parent or adoptive ^ 7) at birth and birth number adoptive or adoptive parents, 7) Law no. 94/1963 Coll., on family, as amended. ". 5. Part of the second, including the heading deleted. 6. Annex no. 3 reads: "Annex no. 3 of Decree no. 296/2004 Coll. Art. II Transitional provisions The form requests for data from the information system issued following the model of the form set out by Decree no. 296/2004 Coll., As amended effective on the effective date hereof, may be used until 31 December 2015.
