1.    Name of Determination
This Determination is the Health Insurance (Patient Episode Initiation) Revocation Determination 2012.
2.    Commencement
This Determination commences on 1 May 2012.
3.    Revocation
This Determination revokes the Health Insurance (Patient Episode Initiation) Determination 2011 No. 1.
